Overview
The Forms feature is only available to districts that use Ecollect, Enrollment, or Enrollment Express.
From the navigation menu, select Forms to review all forms associated with only the selected student and manage all form-related tasks, including reviewing incomplete submissions and verifying completed forms.
Form organization
Forms are organized into two categories:

Enrollment forms

Contains individual forms related to annual re-registration, enrollment, or other student intake activities.

Forms are displayed directly without being part of a packet.

Other forms

Contains form packets, each grouping multiple related forms, such as an Athletics Registration Packet or an Annual Information Update Packet.

Packets are sorted based on the number of incomplete forms. Packets with more incomplete forms are listed first.

Any individual forms not part of a packet are displayed under the General Packet grouping.

Manage forms
The Forms page includes two main sections to help track progress:

Lists all incomplete or in-progress forms and packets.

Displays all forms in rejected, not started, started, or pending review status.

Displays all form packets that contain at least one form with the status of Rejected, Not Started, Started, or Pending Review statuses.

After a form or packet is fully completed, it automatically moves to the Complete section.

Complete section

Lists all forms and packets that are submitted or approved.

Access and submit forms
Select a form name to open the form in a new browser tab. Then, review, complete, and submit your responses.
